Assumptions of Risk and Agreement to Hold Harmless
By registering for any class(es), private training or grooming, you agree to hold Canine Tutor LLC (dba Canine University of Ohio), the trainer and assistants, relations thereto, sponsors of the training, owners of the premises upon which the training is being held and their employees and guests, harmless from any claim for loss, illness or injury which may be alleged to have been directly or indirectly to any dog, person or thing by the act of this dog while in or upon the training premises or grounds or near an entrance thereto, and I personally assume all responsibility and liability for any such claim; and I further agree to hold the aforementioned parties harmless from any claim for loss of this dog by disappearance, theft, damage or injury, be cause or alleged to be caused by the negligence of the aforementioned parties, or by the negligence of any other person, or any other causes or causes. I assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any loss and expense (including legal fees) be reason of the liability imposed by law upon an aforementioned parties for damage because of bodily injuries, including death at any time resulting therefrom, sustained by any person or persons, including myself or on account of damage to property, arising out of or in consequence on my participation in this training, howsoever such injuries, death or damage to property may be caused, and whether or not the same may have been caused or may have been alleged to have been caused by the negligence of the aforementioned parties or any of their employees or agents, or any other persons.
Furthermore, I am aware and accept the fact that by taking my dog to any public facility, park, pet store, vet clinic, groomer or training center such as Canine University of Ohio, my dog could be exposed to viruses and bacteria. I understand that Canine University of Ohio assumes no liability in the unlikely event that my dog contracts an illness, virus or bacterial infection. I take the responsibility of routinely vaccinating my dog as per my veterinarian in order to as best as possible protect my dog. I agree to present such vet records to Canine University of Ohio prior to the first class of each training session my dog attends.
I am aware that Canine University of Ohio and/or the premises where classes are held might have video surveillance 24/7 for safety and security purposes. Video is not used for other purposes without the consent of the individual(s) recorded.
I understand that these Terms and Conditions also apply to Private Training.
I agree that Canine University of Ohio or the trainer has the right to refuse registration or excuse the dog from class for cause which they shall deem to be sufficient. Completion of a registration form for the respective class or private lesson is required before training commences. Furthermore, I have read and fully agree to the following policies and procedures of Canine University of Ohio/Canine Tutor LLC.
Class Policies
A veterinarian health certificate is required before your dog can participate in class. The certificate must show that your dog is current with DHLPP, Rabies, Bordetella (every 6 months) vaccinations and has had a stool check showing a negative result within the past 2 months. Rabies not applicable to puppies under 6 months.
Tuition is non-refundable once class has begun. Class credit will not be given for missed classes.
Dog over 6 months of age should schedule a free class screening. Call for an appointment.
Dogs who bark excessively as well as dogs who show any signs of being aggressive and are potentially dangerous to people or other dogs will not be permitted in classes.
